New Patients


Your first visit to Farmington Valley Dermatology and Surgery establishes the foundation for our relationship with patients.  Patients must fill out certain forms at first appointment. To save time, you can obtain the forms by clicking on the red "Patient Forms" link below, printing, and then filling out in blue or black ink before your appointment.

What to Bring

Photo ID - Driver's license, student ID, senior ID

Insurance Card - If payments are expected to be made through health insurance.

Copay Payment - All copays are to be paid at the time of appointment. We accept VISA, MasterCard, American Express, Discover, personal check, cash.

Completed Patient Forms - If filled out in advance. 

List of Current Medications - Names & doses of current medications.

List of Known Allergies - Names of medications & type of allergic reaction experienced.

Pertinent medical records - Including pertinent notes, labs, and pathology reports from other providers.


What to Expect

Please arrive at least 15 minutes early, so that the check-in process can be completed prior to the scheduled appointment time. This may include completion of any forms not already filled out, entering updated insurance and personal information, etc. 

Minor Patients
Patients under the age of 18 must be accompanied by their parent or legal guardian to every visit.

Arriving Late
We try to accomodate late patients as best we can: If you are more than 15 minutes late to your scheduled appointment time, you may be asked to reschedule for a new appointment. Please phone ahead if you know you will be late. We may still be able to see you but you may have to wait until on-time patients are seen.
